Make a Meaningful Difference in Frailty Care
An exciting opportunity has arisen for a Clinical Fellow to join our dedicated Frailty and Older People’s Medicine Service.
As part of our developing acute frailty model, you will play a vital role in delivering high-quality, patient-centred care for frail older adults. Working within a supportive, consultant-led multidisciplinary team, you will collaborate closely with consultants, specialty doctors, nurses, therapists, and wider healthcare professionals to provide comprehensive assessment and management for patients across the Emergency Department (ED), Same Day Emergency Care (SDEC), and Care of the Elderly wards.
This is an outstanding opportunity for a doctor looking to further develop their clinical expertise in Geriatrics within a progressive service that values innovation, teamwork, continuity of care, and professional growth.
As part of the Acute Frailty Team, you will provide specialist frailty assessment and management within the Emergency Department (ED) and Same Day Emergency Care (SDEC). The role focuses on delivering rapid Comprehensive Geriatric Assessments, identifying appropriate alternatives to hospital admission, and supporting safe early discharge to improve patient outcomes and reduce unnecessary admissions and readmissions.

You will work collaboratively with multidisciplinary teams, community services, primary care clinicians, patients, and families to coordinate ongoing care and ensure continuity across healthcare settings. Responsibilities also include participating in multidisciplinary meetings, clinical governance, audit, teaching, quality improvement initiatives, and undertaking clinical procedures within your scope of practice.
In addition, you will support the education and development of resident doctors and medical students working on the Frailty ward.
