Clinical Lead – Supported Living

Direct Homecare is seeking a compassionate, experienced and clinically confident Clinical Lead to support the development and delivery of our RQIA-regulated supported living services.

This is a key clinical leadership role focused on quality, safety, person-centred practice and positive outcomes for people receiving support in their own homes and communities. The role will work closely with operational managers, frontline teams, families, multidisciplinary professionals and external stakeholders to ensure care and support is delivered to a consistently high standard.

About the Role

As Clinical Lead, you will provide clinical oversight, guidance and support across Direct Homecare’s supported living provision. You will help ensure that people receive safe, effective, compassionate and personalised support, with a strong focus on independence, dignity, safeguarding and quality of life.

This role is ideal for a nurse or experienced clinical professional with a background in learning disability, mental health, complex care, community care, supported living, domiciliary care or social care services.

Key Responsibilities

  • Provide clinical leadership and oversight across supported living services.
  • Support safe, person-centred care planning, risk assessment and outcome-focused support.
  • Advise teams on complex care needs, clinical risk, escalation, safeguarding and best practice.
  • Work collaboratively with operational managers to maintain safe and effective service delivery.
  • Support staff with clinical guidance, coaching, supervision and competency development.
  • Contribute to medication governance, incident review, clinical audits and quality improvement.
  • Promote a culture of dignity, independence, choice, inclusion and positive risk-taking.
  • Liaise with families, Health and Social Care Trust professionals, commissioners, housing partners and multidisciplinary teams.
  • Support preparation for quality reviews, governance meetings and regulatory inspections.
  • Help embed evidence-based practice, reflective learning and continuous improvement across the service.
  • Ensure clinical records, support plans and risk assessments are clear, current and person-centred.

About You

We are looking for someone who combines strong clinical judgement with a genuine commitment to empowering people to live well and independently.

  • Registered nurse qualification or relevant clinical background.
  • Experience supporting people with learning disabilities, autism, mental health needs, complex care needs or behaviours that challenge.
  • Experience in community-based care, supported living, domiciliary care, residential care or similar regulated services.
  • Strong understanding of safeguarding, risk management, care planning and clinical governance.
  • Ability to support and develop frontline care teams.
  • Excellent communication, documentation and problem-solving skills.
  • Confidence working with families, professionals, commissioners and external agencies.
  • A calm, supportive and solution-focused leadership style.
  • Commitment to person-centred practice, human rights, independence and positive outcomes.
  • NMC registration, where applicable.
