Job Overview

Are you an enthusiastic and compassionate Adult Registered Nurse or Paramedic looking for a rewarding role where you can make a real difference? Join our Urgent Community Response Team (UCRT) and help deliver high-quality, patient-centred care in the community, supporting people to remain safely in their own homes and avoiding unnecessary hospital admissions. We are looking for a proactive, supportive band 7 nurse/paramedic with proven clinical leadership and decision-making skills.

UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex—including ambulance services, acute hospitals, GPs, social care, community services and the voluntary sector—we provide holistic assessments within two hours of referral and deliver timely, coordinated care.

As a clinical lead you will be expected to provide senior clinical leadership within the URCT team and undertake a range of clinical duties and nursing procedures along with supporting the team to manage more complex patients within their own home.

You will need to demonstrate expert knowledge surrounding community care and a commitment to provide high quality, safe and effective care for patients requiring rapid nursing intervention within their own home to prevent hospital admission.

Main duties of the job

  • To improve outcomes for people with frailty and complex co-morbidities and reduce the need for these people to attend or be admitted into hospital.
  • Working with partners to ensure effective systems are in place to make best use of services/facilities, aiming to reduce unnecessary admissions, reducing the length of stay in inpatient facilities and more efficient use of community services.
  • Communicating and advocating across areas of clinical responsibility to ensure effective and personalised care is delivered for patients.
  • Organising and managing your own time, allocating/delegating tasks appropriately, in order to achieve service goals
  • Performing comprehensive, specialist, holistic assessment for the individual patients meeting the service criteria. This will include physical examination and exploration of mental state and any cognitive deficits
