Clinical Lead – Neighbourhood Mental Health Team
Band 7 | Permanent
Full Time – 37.5 Hours per Week (Monday–Friday, 9am–5pm)
Flexible Working & Compressed Hours Available

£49,387 – £56,515 per annum (pro rata)

Provide clinical and managerial leadership across community mental health services, helping people transition safely between inpatient and community care while supporting the transformation of mental health services across Sussex.

Who can apply
We welcome applications from experienced registered professionals, including:
- Mental Health Nurses (RMN)
- Occupational Therapists (OT)
- Social Workers
You'll ideally have:
- Significant experience working within adult mental health services
- Experience leading or supervising colleagues within multidisciplinary teams
- Strong clinical assessment and risk management skills
- Excellent organisational and communication skills
- A passion for delivering recovery-focused community care
The role
As a Clinical Lead, you'll provide both clinical and managerial leadership while supporting patients with the most complex needs as they move between inpatient and community services.

Your responsibilities will include:
- Providing clinical leadership across the Neighbourhood Mental Health Teams
- Overseeing complex discharge planning and care pathways
- Supporting timely discharges from inpatient services into the community
- Acting as Lead Practitioner for new admissions not previously known to community services
- Remaining involved from admission through discharge and for 4–6 weeks following discharge
- Carrying out complex priority assessments each week
- Attending ward rounds, MDT meetings, risk huddles, discharge planning meetings, and zoning meetings
- Working closely with Crisis Teams, Acute Day Services, Staying Well Services, and community partners
- Attending CAMHS and Perinatal transition meetings to support seamless care pathways
- Supervising and supporting junior colleagues as part of the leadership team
- Liaising with carers, families, discharge coordinators, and partner organisations
The role plays a vital part in ensuring people receive the right care, in the right place, at the right time.

We're seeking an experienced clinician who can:
- Demonstrate excellent clinical leadership and decision-making
- Support colleagues through supervision, mentoring, and professional development
- Manage complex clinical pathways across multiple services
- Work confidently in a fast-paced, varied environment
- Build effective relationships across multidisciplinary teams and partner organisations
- Prioritise competing demands while maintaining high standards of patient care
- Promote recovery-focused, person-centred practice
You'll also have a good understanding of Health and Social Care Governance and experience managing or supporting staff.
