Clinical Lead – Complex Care & Supported Living
Direct Homecare is seeking a compassionate, experienced and clinically confident Clinical Lead to support the development, governance and delivery of our CQC-regulated complex care and supported living services across England.
This is a key clinical leadership role focused on quality, safety, clinical excellence, person-centred practice and positive outcomes for people receiving support within their own homes and communities. The role will work closely with operational managers, registered managers, frontline teams, families, commissioners, NHS professionals and external partners to ensure care and support is delivered to the highest standards.
About the Role
As Clinical Lead, you will provide clinical leadership, oversight and professional guidance across Direct Homecare's complex care and supported living services. You will support the delivery of safe, effective, caring, responsive and well-led services in line with CQC standards and best practice.
The role focuses on supporting individuals with a range of needs, including:
- Learning disabilities
- Autism
- Mental health needs
- Acquired brain injuries
- Physical disabilities
- Neurological conditions
- Complex health needs
- Tracheostomy care
- Ventilation support
- Enteral feeding (PEG/JEJ)
- Epilepsy management
- Positive behaviour support
- Other complex clinical interventions within community settings
You will play a central role in ensuring people receive personalised care that promotes independence, dignity, choice, inclusion and improved quality of life.
Key Responsibilities
- Provide clinical leadership and oversight across complex care and supported living services.
- Support the assessment, planning, implementation and review of person-centred care and support packages.
- Ensure safe delivery of complex clinical interventions in community and supported living environments.
- Advise teams on clinical decision-making, risk management, safeguarding, escalation and best practice.
- Support Registered Managers and operational leaders to maintain safe, effective and compliant services.
- Lead and support clinical assessments, reviews and transitions into service.
- Develop, monitor and review care plans, clinical protocols and risk assessments to ensure they remain current and effective.
- Provide clinical supervision, coaching, competency assessments and ongoing support to care teams.
- Support training delivery and workforce development across services.
- Contribute to medication governance, clinical audits, incident investigations, lessons learned reviews and quality improvement initiatives.
- Promote evidence-based practice and continuous improvement.
- Work collaboratively with NHS professionals, Integrated Care Boards (ICBs), local authorities, commissioners, case managers, therapists and multidisciplinary teams.
- Build positive relationships with individuals, families, advocates and external stakeholders.
- Support preparation for CQC inspections, internal audits and governance reviews.
- Ensure accurate, contemporaneous and person-centred documentation and record keeping.
- Foster a culture of dignity, respect, inclusion, positive risk-taking and human rights-based care.

About You
We are looking for a clinically experienced professional who combines excellent leadership skills with a genuine passion for supporting people to live fulfilling and independent lives.
Essential Requirements
- Registered Nurse (RGN, RNLD or RMN) with current NMC registration, or equivalent relevant clinical qualification.
- Experience working within complex care, community care, supported living, domiciliary care, learning disability services, mental health services or social care settings.
- Strong understanding of clinical governance, safeguarding, risk management and quality assurance.
- Experience supporting individuals with complex physical, neurological or behavioural support needs.
- Knowledge of CQC regulations, fundamental standards and best practice guidance.
- Ability to support, develop and motivate multidisciplinary and frontline care teams.
- Excellent communication, organisational and problem-solving skills.
- Experience working collaboratively with families, healthcare professionals, commissioners and external agencies.
- Commitment to person-centred care, equality, diversity, inclusion and human rights.

Desirable
- Experience supporting tracheostomy, ventilator-dependent care, PEG feeding or other complex clinical interventions.
- Experience within learning disability, autism or acquired brain injury services.
- Leadership, management or coaching qualification.
- Experience preparing for or supporting CQC inspections.

Salary: Competitive, dependent on experience
Location: England (with travel across services as required)
Reports to: Director of Care / Operations Director
Hours: Full-time, with flexibility to meet service requirements
