Role Description
The post holder will be an integral member of the Hub Bed and Chartridge Multidisciplinary Team (MDT) and provide proactive clinical complex case work for patients within the Community Discharge Hubs, Chartridge Ward, and Interim Health Funded (IHF) beds as required.
Key Responsibilities
- Ensure and promote the timely access to health and social care resources.
- Work in partnership with other MDT and interdependent teams to ensure a safe and timely discharge from the service.
- Provide active case management for patients within the Community Discharge Hubs, Chartridge Ward, and Interim Health Funded (IHF) beds as required.
- Work with a range of professionals to ensure care is appropriate and delivered in a timely manner.
- Demonstrate professional skills and competence in leadership, care planning/co-ordination, medicines management, and care and treatment delivery.
- Provide proactive and responsive support to the Clinical Operational Lead for patients in the Hub Beds, Chartridge Ward, and Interim Health Funded beds (IHF) as required.
- Work across all Hub Beds and where appropriate IHF sites, BHT Hospital and other community hospital sites to meet the needs of an efficient Community Discharge Hub Service.
- Provide interventions tailored to meet the needs of the individual through the case management model.
