Clinical Nurse Specialist for Complex Cancer Late Effects Rehabilitation Service

The Clinical Nurse Specialist for the pain-related Complex Cancer Late Effects Rehabilitation Service provides expert assessment, management, education, and support for people experiencing persistent pain, functional impairment, and other long-term consequences of cancer treatment.

In this role, you will work autonomously within the multidisciplinary team, managing a specialist caseload and coordinating personalized rehabilitation and supportive care pathways.

You will deliver specialist clinical care through face-to-face, virtual, and telephone consultations, providing comprehensive assessment, triage, care planning, and ongoing support.

The role focuses on optimizing symptom management, improving function, promoting self-management, and enhancing quality of life for people living with the consequences of cancer and its treatment.

In addition to clinical responsibilities, the post holder will contribute to service development, education, quality improvement, audit, and research activities, supporting the continued growth and evaluation of Complex Cancer Late Effects Rehabilitation Service and promoting excellence in cancer rehabilitation and survivorship services.

Key Responsibilities

  • Manage an autonomous specialist caseload within CCLERS.
  • Undertake specialist assessment, triage, and care planning for people living with persistent pain, functional impairment, and other long-term consequences of cancer treatment.
  • Coordinate complex patient pathways across primary, secondary, and tertiary care services.
  • Work collaboratively within the multidisciplinary team to support the delivery of high-quality patient-centred care.
  • Act as a specialist resource for patients, carers, and healthcare professionals regarding cancer rehabilitation and survivorship.
  • Support the development, evaluation, and expansion of the CCLERS service, identifying opportunities to improve patient outcomes and service delivery.
  • Contribute to education and training programmes for healthcare professionals, patients, and carers.
  • Promote awareness of the long-term consequences of cancer treatment and available rehabilitation pathways across the Trust and wider healthcare community.
  • Participate in audit, research, quality improvement, and outcome evaluation activities to support service development and evidence-based practice.
  • Maintain accurate clinical records and contribute to the collection of service activity and outcome data.
