The Diabetes Specialist Nurse (DSN)

The Diabetes Specialist Nurse (DSN) will provide expert clinical leadership and specialist diabetes management for housebound patients on the District Nursing (DN) caseload. Working collaboratively with District Nursing teams, General Practitioners, Community Matrons, and other multidisciplinary professionals, the post holder will proactively assess, review, and optimise diabetes care for complex patients in their own homes.

The role aims to improve clinical outcomes, reduce diabetes-related complications, enhance patient self-management where appropriate, and decrease avoidable demand on District Nursing services through specialist intervention, education, and proactive case management. The DSN will support the delivery of high-quality, evidence-based diabetes care while promoting best practice across the community workforce.

Clinical Responsibilities

  • Undertake comprehensive specialist assessments for housebound patients with diabetes on the District Nursing caseload.
  • Provide expert clinical management of patients with complex diabetes needs, including insulin-treated patients and those at risk of poor glycaemic control.
  • Review and optimise diabetes treatment plans in collaboration with GPs, consultants, pharmacists, and community healthcare professionals.
  • Support the safe titration, and monitoring of diabetes medications and insulin therapies within agreed protocols.
  • Identify patients at risk of diabetes-related complications and implement proactive management plans to reduce hospital admissions and prevent deterioration.
  • Lead on the management of diabetic patients on the District Nursing caseload, with the aim of improving patient outcomes, reducing avoidable complications, and decreasing overall demand on District Nursing services through proactive intervention.
  • Provide specialist advice and clinical decision-making support to District Nursing teams regarding diabetes management.
  • Support the management of complex diabetic foot concerns and facilitate appropriate referrals to specialist services.

Education and Support

  • Provide education and training to District Nurses, Health Care Assistants, Community Matrons, and other healthcare professionals to improve diabetes knowledge and competence.
  • Promote best practice and evidence-based diabetes care across the District Nursing service.
  • Support patients, carers, and families with education to improve diabetes self-management and treatment adherence where appropriate.

Service Improvement and Governance

  • Contribute to the development of community-based diabetes pathways and service improvement initiatives.
  • Monitor clinical outcomes and service impact, demonstrating improvements in patient care and reductions in unnecessary healthcare utilisation.
  • Participate in clinical audits, quality improvement projects, and governance activities related to diabetes care.
  • Maintain accurate clinical records and ensure care is delivered in accordance with Trust policies, NICE guidance, and professional standards.

Communication and Partnership Working

  • Act as the specialist diabetes resource for District Nursing teams.
  • Work collaboratively with acute diabetes services, primary care, community teams, and other stakeholders to ensure seamless patient care.
  • Attend multidisciplinary team meetings and contribute specialist diabetes expertise to care planning discussions.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period

This post will be subject to a probationary period.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,870 and a maximum payment of £6,137 per annum, pro rata for part time)
