Job Overview

This is an exciting opportunity to work as part of a progressive and passionate team in Haematology with opportunities to develop skills and knowledge, whilst embedding learning within your role.

The post holder will work as an important member of the Haematology Multidisciplinary Team within Southmead Hospital Operating Unit, collaborating with a range of health care providers in the local community and across the regional Network.

Main Duties of the Job

  • The post holder will work as an important member of the Haematology Multidisciplinary Team within Southmead Hospital Operating Unit, collaborating with a range of health care providers in the local community and across the regional Network.
  • Depending on experience the post holder will provide specialist nursing advice and skills to support patients (in and out patients) and their carers and families throughout their care pathway.
  • The post holder will utilise or be supported in learning the skills of leadership, advanced clinical expertise, specialist education and management to ensure a seamless service for patients, carers and staff.
  • Highly complex advice, support and information will be utilised and/or developed from the time of diagnosis and throughout the curative, palliative and terminal phases. Working closely with the wider multidisciplinary teams in the hospital and the community to ensure a co-ordinated quality service.
  • There are opportunities to develop skills or run nurse led clinics in preassessment where skills of prescribing and assessing patients will be used or can be working towards. You will be supported and expected to participate in education, audit, research, service improvement and patient experience as part of this exciting role.

Detailed Job Description And Main Responsibilities

Interpersonal and Communication Skills

  • To have or working towards Advanced Communication Skills
  • To demonstrate or develop high level skills in order to provide effective communication systems of support for patients
  • To provide patients and their carers (or be working towards) highly specialised advice to manage complex systems
  • To act as a professional member of the team with wider multidisciplinary colleagues and patients and their carers
  • To expertly or working towards delivering highly complex sensitive condition conversations related to information for patients and their carers
  • Provide education and support for patients (or work towards this) receiving a range of specialist treatments
  • If relevant, undertake non-medical prescribing module to support nurse led clinics and reviews
  • To participate (where appropriate) in the planning and delivery of formal and informal specialist education programmes for patients, carers, education forums and education for staff
  • To take personal responsibility for life-long learning and personal development through attending clinical supervision, using the ACCEND framework and appraisal

Professional Knowledge and Expertise

  • RGN Level 1 and current NMC Registration
  • Evidence of Degree level qualification in relevant subject
  • Evidence or working towards Masters Level in relevant subject, or willingness to work towards
  • A relevant teaching qualification
  • Highly developed or developing specialist knowledge underpinned by theory and experience of the care and management of patients with haematological cancers and non haematological conditions including:
    • Acute presentations of malignancy, complications of the disease process and progression
    • Patient pathways and current approaches to patient follow up including My Medical Record
    • Up to date knowledge and understanding of national and local cancer developments

Problem-Solving and Decision-Making

  • To hold, or work towards nurse led clinics including assessment and treatment for patients
  • To have the ability or work towards managing a fluctuating caseload and unpredictable levels of patient referrals and request for specialist advice
  • Experience or working towards the ability in facilitation, negotiation and influencing
  • To support or work towards supporting patients unwell on their treatments
  • Manage daily work within the team, either leading or supporting the wider team

Planning and Coordination

  • To be responsible or support the team in leading and planning for day to day workload
  • To manage or support the team in planning the provision of day to day organisational tasks for self and others with the ability to flexible and adaptable, and to prioritise.
  • To keep accurate and up to date patient records across a number of systems
  • To be responsible or support the development of up to date day to day job plans in order to maximise team efficiency and effectiveness

Practical And Technical Skills

  • Daily use of computer systems
  • Repositioning patients and equipment as necessary
  • Complying with manual handling and health and safety policies and procedures

Patient Care and Support

  • Be familiar or familiarise understanding of protocols to support nurse led clinics

Service Development and Improvement

  • To lead or support on the service development identifying gaps through patient experience or service review through audit and research
  • To collate and record qualitative and quantitative data that provides evidence of productivity, outcomes and quality
  • To promote a culture of evidence based current practice
  • To be involved in health promotion and support
  • To remain up to date with research available for patients in Haematology
  • To gain and review patient experience feedback to provide improvements to service

Finance and Resource Management

  • No budgetary management required for this role
  • Provision of education and management of resourcing for this should be directed to the lead Haematology Clinical Nurse Specialist

Leadership and Team Management

  • To lead or support the team in management of day to day activities and responsibilities
  • To manage or support with creating and maintaining an up to date job plan
  • To be responsible for own learning and remaining current with the use of the ACCEND programme
  • Familiarise yourself with policies and procedures to support yourself and the team
  • Ensure participation in teaching and identifying training needs of self and /or the team and wider colleagues in the speciality

Innovation and Continuous Improvement

  • Take initiative to identify gaps in service review and development
  • Work with and in the team to discuss service improvement and patient experience opportunities
  • Maintain close professional relationships with the research team to support patients with their ongoing care
  • Promote a culture of evidence based practice
  • Seek out or support new innovative ways of working to support the patients’ pathways or treatment modalities

Physical Demands

  • This role requires sitting at a VDU and desk for a prolonged length of time
  • This role will require working in a shared office with frequent and possible interruptions
  • This role will involve repositioning patients, assessing patients physically
  • This role involves use of a telephone headset often for meetings or educational purposes
