Job Description
You will be providing expertise knowledge on the wards and community, as well as supporting a multidisciplinary team. You will also be helping with practice changes within the organisation and ensuring we are updated with the best practices in evidence-based care.
We are offering a great new way of working, independent, autonomous and supporting the patients from admission to discharge where you will be working from 9am-5pm Mondays to Fridays.
Previous experience at Band 6 level in community/acute is essential and you will be required to cover acute and community sites as and when required.
You will be working between sites, covering Guys and St Thomas’ Foundation Trust as well as our community sites in Lambeth & Southwark as and when required.

You will be delivering evidence-based expert nursing care with a high degree of professional autonomy on a daily basis by providing specialist advice to other health professionals and patients.
You will provide regular updates on developments and issues with the service highlighting issues in a timely manner.
You will evaluate practices to ensure interventions are effective, efficient and make a positive difference for patients and colleagues.

The acute Tissue Viability team looks after patients with pressure ulcers, leg ulcers, surgical and traumatic wounds and chronic wounds. The team covers Guys, St Guys & Thomas’ and Evelina and SLAM sites.

The nurse-led team works in partnership with patients, their carers and healthcare professionals to provide expert wound care advice and follow up care.
The team aims to improve and support high standards of practice through clinical consultations, regular audits, development of guidelines and policies and by delivering formal educational training to healthcare professionals.
We believe that training and education are key factors in pressure ulcer prevention.
