Job Description: Clinical Nurse Specialist - Older Persons Crisis Care

We are looking for a dynamic registered practitioner with advanced clinical skills. We want applicants who are keen to be part of transformation and improvement in the area of Older Persons Crisis Care for our Intensive Older Persons Community Team. We require people who can offer strong clinical leadership, education, and mentoring to the team.

Role Overview

Central Norfolk IOPS Team provides a 7-Day a week, 0800-2100hrs service. You will be responsible for supporting the team with triage, providing advice to primary care clinicians where appropriate, prescribing, and assessments and treatment planning. You will be required to support the wider team with risk management and co-ordination of care delivery within a Crisis service. You will be supported with regular supervision and opportunities to work in partnership with other health and social care providers, linking into the wider multi-disciplinary service.

Key Responsibilities

  • Undertaking complex mental health, risk, and capacity assessments.
  • Providing specialist interventions for older adults with dementia, delirium, depression, psychosis, and other acute mental health needs.
  • Supporting crisis resolution and admission avoidance, enabling people to remain safely in their own home or care setting where appropriate.
  • Acting as a senior clinical resource within the multidisciplinary team, advising on complex cases and contributing to clinical decision-making.
  • Supporting effective partnership working with families, carers, primary care, acute hospitals, care homes, and social care services.
  • Providing clinical supervision, training, and development to enhance workforce capability and confidence.
  • Leading on specialist practice relating to older people's mental health, dementia care, and crisis management.
  • Contributing to service development, quality improvement, and transformation initiatives to strengthen community-based crisis care.
  • Supporting the Trust's objectives around reducing inpatient admissions, improving patient flow, and delivering equitable multidisciplinary services for older people.
