The care and support we provide is highly valued by the people we care for and their loved ones, but at present we are only reaching around 10% of dying people at the end of life. Right now, one in four people in the UK with a terminal illness, do not get the care or support they deserve at the end of their lives.
We want a different society than the one we live in now. Our mission for the next five years is to close the gap in the number of people missing out on what they need at the end-of-life.
Job Description
About the service:
Marie Curie is launching the Solihull Identification and Support Service, a targeted, place-based model providing proactive palliative and end-of-life care across six Solihull Primary Care Networks. The service will support earlier identification, stronger end-of-life register reviews, advance care planning, treatment escalation planning, and improved coordination across primary, community and specialist care pathways. By moving from reactive crisis response to earlier community-based support, the service aims to improve outcomes and experiences for patients, families and carers, while contributing to system priorities such as better hospital flow and reduced avoidable urgent care use.
About the role:
We are looking for experienced and compassionate Clinical Nurse Specialists to help deliver this service, supporting earlier identification of people who may be approaching the last year of life and improving access to timely, coordinated, person-centred care.
Working as part of a multidisciplinary and multi-agency model, you will be embedded in primary care MDTs and support GP practices, community services and specialist palliative care partners to identify people earlier, review end-of-life registers, undertake holistic assessment, and support advance care planning, treatment escalation conversations and recording of patient preferences. The role is central to helping people, families and carers receive more planned, coordinated and responsive support closer to home.
This role offers the opportunity to help develop a new place-based service with a clear strategic focus: reaching people earlier, improving the quality and consistency of end-of-life care, strengthening partnership working with Solihull PCNs and local system partners, and using learning from the service to shape future development across Birmingham and Solihull.

Job structure: Permanent, with multiple vacancies available on a full-time or part-time basis
Working hours: Monday to Friday, with typical working patterns of 8am – 4pm or 9am – 5pm
Salary: Marie Curie Clinical Pay Scale Band 6, aligned with AFC: £39,959 – £48,117 per annum
Base: Solihull, community-based across six Primary Care Networks
What you will do:
- Deliver specialist palliative care nursing support as part of the Solihull Identification and Support Service across six Primary Care Networks.
- Support earlier identification of people approaching the last year of life through proactive register review, MDT participation and referral pathway development.
- Undertake holistic assessment, care planning, symptom management and evaluation for people with complex palliative and end-of-life care needs.
- Support advance care planning, treatment escalation planning, ReSPECT conversations and recording of preferred place of care and death where appropriate.
- Work collaboratively with GP practices, primary care MDTs, community services, acute, hospice and specialist palliative care partners to coordinate safe, integrated care.
- Help reduce avoidable crisis presentations, non-elective admissions and acute bed days where community-based alternatives are clinically appropriate.
- Contribute to mobilisation, pathway development, data collection, benefits tracking and routine performance reporting for the commissioned service.
- Use outcome measures, feedback, audit and quality improvement approaches to demonstrate impact and support continuous service development.
What we are looking for:
- Registered Nurse on the NMC register, with a relevant nursing degree and post-registration qualification at Level 6 or above.
- Experience of working in specialist palliative care, ideally within community, primary care or integrated care settings.
- Strong knowledge of complex symptom management, advance care planning, treatment escalation planning and best practice in palliative and end-of-life care.
- Ability to work autonomously, manage a caseload, assess complex needs and influence multidisciplinary decision-making.
- Excellent communication skills, including confidence in supporting sensitive conversations with patients, families, carers and professionals.
- Experience of working collaboratively with multiple partners and services to improve coordination, equity of access and patient experience.
- A compassionate, values-led and person-centred approach, with a clear commitment to advocating for patients and those important to them.
- A flexible approach to working and the ability to travel across Solihull community locations as required.
