Role Description
The Clinical Pharmacist is a full-time, on-site role based in Evesham, working as part of a multidisciplinary primary care team. The role involves providing clinical medication reviews, optimizing pharmaceutical care plans, and supporting safe, evidence-based prescribing across GP practices. The Clinical Pharmacist will collaborate closely with GPs, nurses, and other healthcare professionals to resolve medication-related issues, improve adherence, and ensure compliance with NHS guidelines and local protocols. Additional tasks include:
- Supporting medicines reconciliation
- Contributing to quality improvement initiatives
- Educating patients and staff on safe medicine use
- Maintaining accurate clinical documentation
Qualifications
- Demonstrated competence in Pharmacy Practice and Pharmacy, with the ability to work safely and effectively in a clinical primary care setting.
- Strong knowledge of Pharmaceutical Care and Medicine, including evidence-based prescribing, therapeutics, and medication review.
- Experience with Compounding and preparation of medications in accordance with regulatory and professional standards.
- Registration as a Pharmacist with the General Pharmaceutical Council (GPhC) and eligibility to work in the UK.
- Previous experience as a Clinical Pharmacist in primary care, hospital, or community settings is highly beneficial.
- Excellent clinical judgment, attention to detail, and ability to interpret guidelines, formularies, and prescribing policies.
- Strong communication and consultation skills, with the ability to explain treatment plans and medicines to patients in an accessible way.
- Proficiency in using electronic health record systems and digital tools for documentation, audit, and reporting.
- Ability to work collaboratively within multidisciplinary teams and manage a clinical caseload independently.
- Commitment to continuous professional development, quality improvement, and adherence to NHS governance and safety standards.
