Job Role: Clinical Pharmacist – Independent Prescriber (Mental Health)
Location: GP Pathfinder Clinics (Utopia House, 206 High Road, Willesden, NW10 2PB)
Contract Type: Self-employed
Hours: Full-time - On-site
Salary: £28–£30 per hour

Job Overview
We are seeking a motivated Clinical Pharmacist – Independent Prescriber with mental health experience to join our network of GP practices across Brent. The successful candidate will work closely with GPs, nurses and other healthcare professionals to provide clinical expertise in medicines management, with a particular focus on supporting patients with mental health conditions and optimising the use of mental health medicines within primary care.
This is a key role in supporting safe prescribing, improving patient outcomes and delivering high-quality, patient-centred care.
Key Responsibilities
- Conduct comprehensive clinical medication reviews and support the optimisation of medicines for patients with long-term conditions and complex needs.
- Review and optimise psychotropic medicines, including antidepressants, antipsychotics and mood stabilisers.
- Monitor the effectiveness, side effects, interactions and safety of mental health medicines and arrange appropriate follow-up.
- Support patients with complex mental health needs, including medication adherence and shared decision-making.
- Manage repeat prescriptions and medicines reconciliation, including post-discharge follow-up.
- Deliver patient-facing consultations and provide appropriate advice regarding medicines and treatment.
- Work closely with GPs, nurses and the wider multidisciplinary team to support safe and effective mental health treatment.
- Apply NICE guidance and evidence-based practice in clinical decision-making.
- Participate in audits, QOF and medicines safety initiatives.
- Identify medication-related risks and support the safe management of high-risk medicines.
- Support continuity of care by liaising with community pharmacies and other healthcare professionals where required.
- Maintain accurate clinical records and comply with clinical governance and CQC requirements.
- Contribute to the education and support of junior pharmacists and other clinical staff where appropriate.

Skills We Are Looking For
The successful candidate will be able to:
- Work autonomously as an Independent Prescriber.
- Demonstrate experience working with patients with mental health conditions, preferably within primary care.
- Demonstrate good knowledge of psychotropic medicines and their monitoring and optimisation.
- Conduct structured medication reviews and make appropriate evidence-based prescribing recommendations.
- Apply NICE guidance in clinical decision-making.
- Communicate effectively with patients, particularly when discussing sensitive mental health issues.
- Work effectively with GPs and the wider multidisciplinary team.
- Identify medication safety issues and reduce prescribing risks.
- Prioritise workload and manage clinics independently.
- Confidently use EMIS.

Essential Requirements
- MPharm degree (or equivalent).
- GPhC registration.
- Independent Prescriber qualification.
- Experience working with patients with mental health conditions and/or mental health medicines.
- Experience working as a Clinical/Mental Health Pharmacist within a GP or primary care setting.
- Good understanding of psychotropic medicines and their monitoring.
- EMIS experience.
- CPPE Primary Care Pathway enrolment or completion.
- Excellent clinical, communication and IT skills.
Desirable
- Experience supporting patients with complex mental health needs.
- Experience in medicines optimisation and patient counselling.
- Understanding of mental-health-related safeguarding, capacity and risk considerations.
- Experience working collaboratively within a multidisciplinary team.
