CLINICAL PRACTICE PHARMACIST – DERBY
£28 per hour
Full time or Part time
Your responsibilities will include:
- Structured medication reviews
- Supporting patients with long-term conditions
- Polypharmacy and complex patient reviews
- Repeat prescribing & reauthorisation
- Medication reviews following hospital discharge
- Pharmacist-led clinics
- Prescribing safety and medicines optimisation
- Clinical audits & quality improvement
- Supporting NICE guidance and local formularies
- Reviewing pathology results and medication monitoring
- Working closely with GPs and the wider PCN pharmacy team
- Supporting patients with adherence and medicines education

Essential Requirements
- Pharmacist qualification
- Independent Prescriber qualification
- Previous experience working within a GP practice
- Minimum 2 years' post-qualification experience
- Experience supporting long-term conditions/chronic disease management
- Good understanding of primary care prescribing and medicines optimisation
- Strong communication and organisational skills
- Ability to work independently and as part of an MDT
