Registered Mental Health Practitioner - Crisis Access Hub
We are looking for a registered mental health practitioner to join the new Crisis Access Hub as a Band 6 Clinical Screening Practitioner. This is a key role in the Crisis Pathway Redesign Programme, supporting timely and consistent access to the right crisis support across Kingston, Merton, Richmond, Sutton and Wandsworth.
Role Overview
The role is focused on telephone and electronic referral screening, prioritisation, signposting, and onward routing. It does not include face-to-face assessment, home visits, or treatment. You will work within agreed clinical frameworks, using judgement to identify risk, urgency, and the most appropriate pathway, with escalation to Band 7 where required.
