We have an exciting opportunity to join our All-Age Complex and Continuing Care (AACCC) service as a Band 6 Clinical Practitioner, supporting children, young people and adults with some of the most complex health and care needs across Thames Valley.
As a Clinical Practitioner, you'll play a vital role in ensuring people receive the right care, in the right place, at the right time. Working closely with individuals, their families and a wide range of health and social care professionals, you'll use your clinical expertise to assess complex needs, support person-centred decisions and help people achieve the best possible quality of life.

Main duties of the job
As a Clinical Practitioner within our All-Age Complex and Continuing Care (AACCC) service, you'll use your clinical knowledge and experience to make a real difference to people living with complex health and care needs. This is an opportunity to take your nursing career in a new direction, combining clinical assessment, professional judgement and collaborative working to help ensure people receive the right care and support.
At the heart of your role will be undertaking NHS Continuing Healthcare (CHC) assessments and reviews, working closely with individuals, their families and multidisciplinary teams to understand their needs and make evidence-based recommendations in line with the National Framework. You'll also have opportunities to develop your experience across the wider CHC pathway, including Fast Track, care planning, clinical commissioning, case management and personal health budgets.
No two days will be quite the same. You'll work with people in a variety of settings, navigate complex clinical situations and bring information to influence different professionals to support person-centred decision making. You'll advocate for individuals, work collaboratively with health and social care partners and use your clinical expertise to help ensure care is safe, appropriate and responsive to changing needs.

Detailed Job Description And Main Responsibilities
You don't need previous experience in Continuing Healthcare. We recognise the wealth of knowledge and skills that registered professionals inclusive of Allied Health Professionals/Occupational Therapists, bring from different clinical backgrounds, whether that's acute care, community nursing, mental health or another clinical setting.
This is a fantastic progression opportunity for new and existing colleagues and those looking to develop specialist expertise in complex case management and clinical commissioning. You'll be supported to build your knowledge and confidence while working alongside experienced colleagues who share your commitment to delivering excellent care.
With 9am–5pm core hours, flexible working patterns and opportunities to work remotely, this role offers an attractive alternative to traditional shift-based working, combining clinical autonomy, professional development and a better work-life balance.

Qualifications
Essential criteria
Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area with up-to-date registration with the relevant regulatory body e.g. Nursing & Midwifery Council (NMC).
Experience
Essential criteria
Previous experience of working in a health or care environment (including acute, community, mental health, local authority etc. and evidence of continuing professional development Experience and understanding of health and social care environment and roles and responsibilities within it.
Desirable criteria
Further training or significant experience in project management and planning, financial management or supporting change management processes.
Skills
Essential criteria
Comprehensive knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care Knowledge and experience of working with Safeguarding issues and Policy guidance Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts
