Clinical Quality & Governance Lead Community Services
Permanent (26.25 hours)
The Quality Governance & Quality Improvement Lead provides facilitation and coordination to embed robust quality governance and continuous quality improvement (QI) across community services. The post holder will ensure that systems, processes and organisational culture consistently support the delivery of safe, effective and person-centred care within community and home-based settings.
The role has a dual focus: supporting delivery both Quality Governance agenda, providing facilitation/coordination in relation to patient safety, risk management, complaints, clinical audit and regulatory and policy compliance to support continuous learning and improvement.
Responsibilities
- To support community services to develop safe, high-quality models of care. Promoting Quality Improvement.
- Facilitating and delivering QI programmes and projects that improve Patient outcomes, Safety and improve experience of care within community services.
- By building QI capability across teams and promoting a culture of improvement.
- By using data, feedback and evidence to drive measurable improvements.
- Support the design, delivery and evaluation of quality improvement programmes and projects across community services.
- Apply recognised QI methodologies (e.g. Model for Improvement, PDSA cycles, measurement for improvement).
- Identify improvement priorities using data from incidents, audits, complaints, patient feedback and outcomes.
- Support spread and sustainability of improvement across teams and neighbourhoods.
- Coordinate quality governance arrangements across community services.
- Ensure effective systems to support timely incident reporting, learning responses, complaints, claims and risk management.
- Support the Patient Safety Incident Response Framework (PSIRF) within community settings.
- Support the maintenance and review of community risk registers, ensuring risks are reviewed, mitigated and escalated appropriately.
- Prepare governance reports and information for review by senior leaders and committees.
