Purpose of the Role
Hold clinical safety assurance for a live, growing digital estate, ensuring the systems we build and the systems we buy are demonstrably safe for the people using them.
Sits within: Clinical Safety.
Works closely with: Product, Engineering, Clinical, Governance.
Primary impact:
- Take named Clinical Safety Officer responsibility for part of our multi-system estate, under DCB0129 where we manufacture and DCB0160 where we deploy, and carry each system through to an approved safety case.
- Lead the hazard identification and risk scoring
- Support our evolving thinking on how to use AI for clinical safety

- You can run a hazard workshop with clinicians, developers and operations in the room and come out with scored hazards, not a list of worries.

What This Job Is
- Hands-on. You write the plans, build the hazard logs, run the workshops and draft the reports yourself.

What This Job Isn’t
- A documentation exercise. A compliant-looking artefact set that doesn’t reflect the real system is the failure mode, not the goal.
- A policy or audit role at a distance from delivery.
Core Duties and Responsibilities
Clinical risk management delivery
- Act as named CSO for an allocated portfolio, across our manufacturer (DCB0129) and deploying (DCB0160) roles.
- Produce and maintain the Clinical Risk Management Plan, Hazard Log and Clinical Safety Case Report for each system.
- Facilitate multidisciplinary hazard identification and risk scoring workshops, and keep the log current between them.
- Specify controls, agree named owners, and verify implementation and effectiveness separately before go-live.
Assurance Of What We Buy And What We Build
- Assess supplier safety evidence: DCB0129, DTAC, device status where relevant and state what is held, what is missing and what risk that leaves.
- Engage Product and Engineering from concept, not at release: in the room before release scope is agreed, reassessing hazards as the build moves.
- Input to business cases, procurement and new-system triage, so scoping happens before deployment.
Safety in live service
- Review safety incidents, assess their impact on the affected safety case, and maintain the Safety Incident Management Log.
- Assess changes proportionately and reissue a safety case report wherever clinical risk changes.
- Keep the estate register and the monthly safety position accurate enough to act on.
AI-augmented clinical safety practice
- Use AI to accelerate and improve our clinical safety work.
- Treat everything AI produces as candidate analysis, subject to your review and multidisciplinary validation before it enters an approved artefact. Making that boundary auditable is part of the job.
- Contribute to assurance of AI-enabled products: such as an AI scribe in live clinical use and maintaining the Clinical AI Risk Register.
Optional Bonus Responsibilities
- Deputy Clinical Safety Officer cover, so documentation can be approved when the Director is unavailable.
- Supporting clinical safety for our India operation, where the regulatory position differs.
To Thrive in this Role, You’ll need
Core Skills And Qualifications
- A registered clinician with current registration with an appropriate professional body (GMC, NMC, HCPC or equivalent).
- Accredited Clinical Safety Officer training
Experience
- Risk management experience applied to clinical domains: hazard identification, severity and likelihood, risk acceptability, control verification
- Hands-on production of DCB0129/0160 artefacts: not only reviewing or commissioning them and facilitating hazard workshops with mixed clinical and technical groups.
- Experience of agile or fast-moving delivery, where the system changes continually.
- Writing an exec can act on: someone accepting residual risk should understand exactly what they’re accepting.

- Digital mental health or neurodevelopmental services; AI-enabled health products; being the first or only safety person somewhere; medical device regulation as a regime separate from the DCB standards.

What Success Looks Like
Success measures
- named CSO for an agreed portfolio, each system walked yourself, at least two hazard workshops run and scored against the approved acceptability criteria.
- complete, approved artefact sets for your highest-criticality systems. Any supplier gaps resolved or escalated with the residual risk written down.
- CSO involvement recorded before release scope is agreed on new work.
