About the Role
This is a multi-classed position, suitable for either a Registered Nurse Level 2 or Allied Health Professional Level 2. Appointment to this position will be commensurate with the applicant’s level of professional experience, demonstrated competence, and capacity to perform with increasing independence, clinical judgement, and complexity of practice as outlined in the role requirements.
What Are We Offering?
We are offering a short-term opportunity to join the Regional Operations Centre (ROC) Community Services Team and make a meaningful difference to the lives of regional patients by supporting their transition from metropolitan hospitals to care closer to home. As part of a collaborative and multidisciplinary team, you will contribute to the coordination of community service referrals and Transitional Care Programs (TCP), helping to ensure patients receive the right care at the right time and in the most appropriate setting.
This position offers the opportunity to further develop your leadership skills in a supportive environment while playing a key role in delivering high-quality, patient-centred care across South Australia's health system, continually building your skills, networks, and professional practice.

- A proactive and organised professional with experience coordinating care, managing competing priorities, and facilitating smooth and timely patient transitions across healthcare settings.
- A motivated clinician with strong problem-solving, decision-making, and organisational skills who is committed to quality improvement, continuity of care, and delivering effective healthcare services for regional communities.
- A collaborative team player with strong communication skills, able to build effective working relationships across multidisciplinary teams, healthcare providers, patients, families, and community agencies.

How Will You Contribute?
You will...
- Coordinate safe and timely patient transitions by assessing, triaging and managing referrals, facilitating access to community services and Transition Care Program (TCP) packages, and supporting regional patients to receive care closer to home.
- Apply your clinical expertise and professional judgement to manage complex care coordination requirements, identify appropriate service options, and deliver person-centred outcomes that meet individual clinical and community care needs.
- Build and maintain strong stakeholder partnerships with metropolitan and regional health services, community providers, aged care services and multidisciplinary teams to support integrated care and seamless patient journeys.
- Support service quality, improvement and operational excellence through accurate documentation, data management, reporting, service evaluation, and participation in projects, committees and initiatives that enhance ROC Community Services.
- Contribute to a collaborative and supportive team culture by sharing knowledge, providing guidance to colleagues, participating in professional development activities, and promoting best-practice approaches to care coordination and service delivery.
