About The Role

We are recruiting to Clinical Support Workers in our Community Nursing and Integrated Care Teams in Wakefield. Further details about each of these roles is in the sections below.
For Both Roles It Is Essential That You Have
- Full driving licence.
- Access to and use of your own vehicle for work purposes.
- An ability to work independently in community settings while supporting the wider multidisciplinary team.
- Level 2 in numeracy and literacy (or equivalent)
Plus One Of The Following
- Vocational Level 3 qualification in a Health related subject regulated by OFQUAL
- For existing Mid Yorkshire staff evidence of Band 2 competency booklet completed
- Any new employees who can evidence competency of core clinical skills, as outlined above in the CSW Band 3 JD

Community Nursing Team
Adult Community Service is a city-wide service that provides Community Nursing care closer to home, making a difference to our patients and the lives of their carers and relatives.
Our Clinical Support workers work in our multidisciplinary teams who support our patients by assisting the Registered Nurses in delivering care. This includes Wound care, Planned Treatments and tests, in both care homes and patients own homes.
As Clinical support worker, you will play a vital role in supporting patients within their own homes and community settings. Working alongside registered professionals, you will help provide compassionate, person-centred care that promotes independence and wellbeing.
This will require you:
- To be able to communicate effectively with a broad range of individuals, both verbally and in writing.
- To share our commitment to maintaining high standards in everything we do.
- To demonstrate that you can work effectively within a team that behaves in accordance with our values.

Integrated Care Team
The Clinical Support Worker role within the Integrated Care Team is an integral part of the service, providing patient centred care to the residents of Wakefield and the five towns area within their preferred place of care.
Key Responsibilities
- Deliver holistic, safe, and effective care in line with Trust values and behaviours.
- Support nurses and other healthcare professionals in providing patient care.
- Provide personal care to Palliative Care and Rehabilitation (Rehab) patients in their own homes, including:
- Bathing
- Toileting
- Dressing
- Repositioning patients
- Assisting with personal hygiene and appearance
Working Requirements
The Integrated Care Team operates 24 hours a day, 7 days a week. You must be willing and able to work:
- Days and nights
- Weekends and bank holidays as required
The role involves significant travel between patients' homes.

- Ability to provide dignified personal care to patients with a range of complex needs.
This role would suit someone who enjoys working directly with patients in the community, is comfortable providing personal care, and has the flexibility to work a variety of shifts across a 24/7 service.
