As a Clinical Triage Practitioner within our Community Health Single Point of Access (SPA), you will play a pivotal role in ensuring patients receive the right care, in the right place, at the right time. Using your advanced clinical judgement and knowledge of community and urgent care services, you will help prevent unnecessary hospital admissions, support safe and timely discharges, and improve outcomes for patients across our communities.

About The Role
As a Clinical Triage Practitioner, you will provide expert telephone-based clinical triage and assessment within the Community Health Single Point of Access. Working autonomously as part of a multidisciplinary team, you will assess referrals from health and social care professionals, determine clinical priority, and coordinate access to appropriate community services.
You will act as a key link across health, social care and voluntary sector services, ensuring patients are directed to the most appropriate support while promoting independence and wellbeing within the community.
Through your clinical expertise and decision-making skills, you will contribute to admission avoidance initiatives, facilitate safe and effective discharges from acute settings, and ensure timely access to community-based interventions.
Location: Unit 2, The Industrial Quarter, Foxcote Avenue, Peasedown St. John, Bath, BA2 8SF
Hours: 30.5 hours per week

Key Responsibilities
- Deliver high-quality telephone clinical triage and assessment within the SPA
- Use sound clinical judgement and diagnostic reasoning to provide treatment advice, care guidance or onward referral in line with agreed pathways
- Support admission avoidance and facilitate safe and timely discharge through coordinated referrals into community services
- Help patients navigate the wider health and social care system, providing effective signposting to VCSE organisations and community resources
- Promote independence and wellbeing by supporting people to remain safely at home wherever possible
- Work collaboratively across health and social care partners, contributing to a fully integrated approach to care
- Participate in a variety of shifts across extended hours, ensuring the SPA remains responsive to professionals across the system
Essential
- Registered healthcare professional with current professional registration
- Evidence of ongoing CPD relevant to professional background
- Current Basic Life Support (BLS) certification (minimum)
- Experience working independently within a relevant clinical setting
- Experience delivering urgent care or support
- Proven ability to work across organisational boundaries within health and social care
- Experience providing professional telephone-based clinical support
Desirable
- Completion of physical assessment and clinical reasoning training
- Remote triage qualification or experience
- Experience using physical assessment and clinical reasoning with adults
- Experience in mental health assessment and management
- Experience as a clinical supervisor
- Knowledge of NHS data capture requirements (e.g. RTT)
- Experience working to targets and performance measures
- Knowledge of local services and pathways
- Experience in person-centred planning
- Demonstrable leadership skills
- Experience supporting service change, improvement or transformation
