🌟 We’re Hiring! Community Care Coordinator Permanent, Full Time Bristol

7am - 10pm inc. Weekends and Bank Holidays on a Shift Basis Competitive Salary + Attractive Uplift for Out of Hours

Do you enjoy working in a fast-paced, people-focused role where you can make a real difference to clients, care staff, and service users?

We are looking for a proactive and compassionate person to join our team and provide an excellent care experience by helping to ensure the quality and timely coordination of ongoing complex care packages.

đź’ˇ About the Role

In this role, you will:

  • Act as the initial point of contact for clients, candidates, and service users by fulfilling routine and complex care requests.
  • Arrange the very best healthcare professionals to deliver quality care as per our client's health and social care needs.
  • Take ownership of rotas and accountability of filling available shifts.
  • Ensure CQC compliance and administration for care packages is relevant and up to date.
  • Attend weekly assessment meetings to ensure effective management and execution of quality care.
  • Be responsible for ongoing planning, monitoring, and reporting on packages of care.
