Job Introduction

Job Title: Community Care Officer

Starting Salary: £32,578 progressing to £35,570 per annum

Hours: 37 hours per week

Location: Hertfordshire

Contract Type: Permanent | Fixed Term (see below)

Directorate: Adult Care Service

About The Role

You will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005.

We seek CCO's who excel at engaging with people, promoting inclusive community options, and supporting a self-directed approach while maximising universal services. Our ideal candidates are resilient, robust, imaginative, and reflective. As a small team, we collaborate cohesively to achieve our outcomes, and we’re committed to smart and flexible working.

About The Teams

Older People Service West

Watford & Three Rivers Locality Team

37 hours per week | Permanent

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.

Integrated Discharge Service

Watford General Hospital

37 hours per week | Permanent

The Integrated Discharge Team at Watford General Hospital support a 7 day discharge service. This is a 37 hour a week position, and 1 in every 8 weekends you would be expected to work a Saturday and Sunday.

Our work is fast paced as we focus on supporting people to leave hospital as quickly and safely as possible, with a person-centred approach, with the majority of the work being through Discharge to Assess.

The usual hours are 9am – 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role.

About You

Essential criteria

  • The team is community based and you must have a full UK driving licence and regular access to a vehicle for working purposes
  • Experience of working in social care
