Join Nottinghamshire County Council in the Preparing for Adulthood (PFA) Team, a dedicated team committed to making a positive impact in our community. This is an exciting opportunity to become part of PFA (adult Social Care), a multi-specialist team offering assessment and support planning for young people as they transition into adulthood. As a Community Care Officer, you will work in a strength-based way, aiming to make a real difference in the lives of young people. You will collaborate with various partners, including Children’s Social Care, education, and health services, to support young individuals in achieving their aspirations during this pivotal time in their lives.
Key Responsibilities
- Conducting assessments and developing support plans tailored to individual needs.
- Collaborating with young people and their families to identify goals and aspirations.
- Working alongside multi-agency teams to ensure seamless transitions into adult services.
- Maintaining accurate records and preparing reports to track progress.
- Participating in safeguarding activities and promoting the well-being of young people.
- Engaging in continuous professional development and training opportunities.

To Succeed In This Role, You Will Need
- A relevant health or social care qualification (NVQ Level 3 or equivalent).
- A minimum of two years’ experience in a care-related role, either paid or voluntary.
- Knowledge of community care services and relevant legislation, including the Care Act 2014.
- Proficiency in using IT applications for record-keeping and reporting.
