About the Role

We are looking for enthusiastic Community Care Officers to join our Urgent and Emergency Care (UEC) Service in the South of the County. This is an exciting opportunity to work in a dynamic team, helping individuals transition safely from hospital to home. You will play a crucial role in supporting people to live independently and achieve their personal goals.
Key Responsibilities
- Attending Multi-Disciplinary Team (MDT) meetings to assist with discharge planning.
- Conducting assessments and reviews in homes and care units, focusing on strengths-based approaches.
- Supporting carers and fostering professional relationships.
- Contributing to a duty rota, including some weekend work to meet service needs.
- Collaborating with colleagues and managers to achieve timely outcomes.

Requirements
- A relevant health or social care qualification (e.g., NVQ Level 3 or equivalent).
- Knowledge of community care services and legislation related to adult social care.
- Proficiency in using IT applications for case management and reporting.
