An exciting opportunity has become available for a skilled, knowledgeable, and forward-thinking Diabetes Specialist Nurse to join our Community Diabetes Service, covering the Gloucester locality.
Based at Southgate Moorings, you will be part of a dedicated community-based diabetes team working within primary care. The role involves close collaboration with healthcare professionals across General Practice and Community Hospitals to support and enhance diabetes care delivery.
About the Role
We are seeking a motivated and enthusiastic registered nurse with a recognised diabetes qualification and recent diabetes experience. Experience working within primary care and possession of a Non-Medical Independent Prescribing qualification would be advantageous.
The successful candidate will be confident working independently, possess excellent communication and interpersonal skills, and be able to travel throughout the locality. A full driving licence and access to a vehicle are therefore essential.
Key Responsibilities
- Managing a diverse community-based caseload, predominantly supporting patients with Type 2 diabetes.
- Care delivery across a variety of settings, including community hospitals, GP practices, outpatient clinics, and patients' homes.
- Providing patient education and supporting self-management.
- Providing education and training to healthcare professionals.
- Participating in mentored/virtual clinics with primary care staff.
