The Community Engagement & Partnership Lead role is responsible for the implementation and delivery of the Community Engagement & Partnership strategy agreed between the Service Manager and Commissioners; and for working closely with the wider delivery team to support the achievement of Key Performance Indicators (KPIs), and other objectives, across the contract.
As the Community Engagement & Partnership Lead you will be responsible for achieving the partnership requirements of the contract with a specific responsibility for place-based and asset-based community and stakeholder engagement in ways that maximise referrals and achieve the activation of healthy behaviour change.
Responsibilities
Implement, and deliver the Community Engagement & Partnership Strategy to:
- Encourage appropriate referrals into the services via professional and self-referral channels
- Utilise an Asset Based Community Development approach that leverages and harnesses the richness of skills, experiences, and opportunities from across the areas and within communities
- Work with key referring pathways to establish innovative engagement techniques that ensure referrals are steady and targeted to key areas and priority groups
- Deploy our marketing and comms efforts to the right locations, via the right medium that ensure efficient, and effective use of resources
- Act as a conduit to the wider team from the referral network to ensure our approaches compliment and support active referring habits
- Ensure co-production opportunities are maximised
Develop and establish effective referral pathways into and out of the service:
- Working collaboratively with staff and partner agencies in a coordinated way to reach out and maximise referral pathway opportunities
- Ensure all professional referral routes into the service are simple, clear, and friction-free
- Make sure all referrers understand how to refer, eligibility criteria and scope of service
- Lead on the development and review of outward referral pathways to ensure safe and effective support to service users/participants when requiring additional services
Establish strong and trusting relationships with a portfolio of wider services / organisations, to provide holistic support to individuals, families, and communities, including (but not limited to), employment services, financial support services, housing options and lifelong learning. Actively seek membership in local networks and meetings.
Support key referring stakeholders with learning opportunities so they can maximise their opportunity to make their contact count with individuals and build confidence to support and advocate lifestyle behaviour change

- Innovative referral strategies, complete with measures of effectiveness
- Capture of learning opportunities deployed across the network
Develop and deliver presentations, engaging pop-ups, and stands across the wider network and communities to promote services, including working with the Marketing and Communications Team to produce tailored resources to promote programmes and services, including:
- Attendance at GP Practice protected learning time
- Lifestyle related forums and networks
- Community events and fun days
Support in the effective development of high performing teams:
- Support all to access supervision and ensure all engage in the development of annual Personal Development Plans (PDPs)
- Support the Service Manager in the recruitment, onboarding and induction of team members as required
- Create a culture of further development and constructive feedback with individuals and teams, embedding and ensure behaviours demonstrate the company values
- Working with other team members to harness, capture, and learn from insight and intelligence sourced proactively and informally via referring networks, partners, communities, and individuals to support continuous improvement and scale service reach and engagement.

Qualifications & Experience
Essential:
- Line management experience, including management of remote teams
- Demonstrable influencing skills that promote commitment and action
- Demonstrable experience of implementing and managing a similar service
- Demonstrable experience of achieving and exceeding targets
- Extensive experience of developing, and maintaining partnership relationships (internal and external)
- Ability to travel independently within an agreed geography
- Experience of working in a health or social care environment or good understanding of the issues around maintaining a healthy lifestyle
- Experience of creating and deploying a strategy
- Experience of report writing and communication of data and outcomes
- Experience working with community partners and stakeholders
Desirable
- Experience of working with health or social care professionals or stakeholders
- Experience of working in services that support lifestyle behaviour change

- A strong understanding of working with communities, especially those communities experiencing increased barriers and challenges with their health and wellbeing
- A confident understand of the wider and social determinant of health
- Excellent internal and external stakeholder engagement and management
- A strong track record of interpersonal relationships including the ability to build, develop and maintain relationships.
- Forward thinker with the ability to quickly identify and address challenges.
- Strong negotiation skills to resolve any issues

- Excellent communication skills with the ability to adapt to a wide range of communication and learning styles

Additional Information
This is a community based role, it requires 70% face to face engagement and around 30% home working, however this could be subject to change to meet business needs.

Salary
- Minimum: £28,000.00
- Maximum: £32,000.00
