Your New Role
We are looking for an organised and collaborative individual to act as the key operational link between Portsmouth City Council Adult Social Care, the outsourced Community Equipment Service (CES) provider, and equipment prescribers across health and social care in Portsmouth.
This is an exciting opportunity to play a pivotal role in ensuring the effective delivery, performance, and continuous improvement of the Community Equipment Service. The role will provide administration support and coordination of responses to prescriber issues. You will work closely with commissioners, health partners, prescribers, and service providers to support high-quality equipment provision that promotes independence and wellbeing for local residents.

The current Community Equipment Service contract is jointly commissioned with Southampton City Council, and you will work collaboratively with colleagues in Southampton to support shared priorities and deliver key operational objectives.
Who Is The Person Summary
We are looking for an organised individual with experience working within health and/or adult social care services, ideally in partnership with multidisciplinary teams and external stakeholders. You will have a good understanding of community equipment services and the important role they play in supporting independence, alongside an awareness of integrated working across health and social care.
You will be confident in interpreting performance data and using insight to support service improvement, with strong analytical, communication and stakeholder engagement skills. The role requires someone who can work independently, manage competing priorities and take a solution-focused approach to challenges. You will also be comfortable chairing meetings, facilitating collaborative discussions and working autonomously with limited supervision.

Strong organisational skills, attention to detail and proficiency in IT systems, including Excel, reporting tools and communication platforms, are essential. An understanding of operational governance within commissioned or outsourced services would be advantageous.
