We are recruiting a Community Health Care Support Worker for our Urgent Community Response team in Hillingdon and we would love to receive your application!
Are you enthusiastic, compassionate and committed in delivering good quality, patient-centred care to patients in the community? Would you like to be part of our well led, supportive and highly clinically skilled Urgent Community Response Team based in the borough of Hillingdon?
We are looking for Health Care Support Workers who are caring, committed and motivated to work within our community nursing teams. As an HCSW you will play a crucial role in supporting the registered nurses and other allied healthcare professionals in delivering high quality, clinical care and rehabilitation with patients in their own homes.
Main duties of the job
You will be a valued member of the community team and receive training and support on a multitude of nursing clinical skills, in order to practice confidently and safely within the community. These skills comprise of:
- Venepuncture
- Support nursing visits
- Wound care management
- Providing care calls in the community
- Catheterisations (female and suprapubic)
- Basic life support

You need to be a confident individual who is able to build good relationships with patients, their families and carers; and work in partnership with our multi-disciplinary Integrated Care Team. We are looking for people with previous experience of caring and ideally you will have a care qualification such as NVQ/QCF Level 3 or be able to demonstrate considerable experience in this field. We would expect you to be competent to work on your own in patient's homes, under the supervision of nurses and expect that you will have some clinical skills and knowledge. We would expect you to be able to have a full UK driving licence with access to a car enabling yourself to travel around the borough of Hillingdon.
Working for our organisation
Our Hillingdon Urgent Community Response Team operates 7 days a week and you will be expected to work on a rota system, (08.00hrs – 22.00hrs) including weekends on a rota.
In return for your commitment to the Hillingdon Urgent Community Response service, you will receive all of the support and development you need to consolidate your knowledge and skills which will prepare you for professional progression within the nursing profession. We will also provide access to continuing professional development resources which will support you to complete modules to help you develop a clinical speciality aligned to the needs of the service. We are looking for a Health Care Support Worker for 37.5 hours a week who is caring, committed and motivated to work within our community nursing teams. As an HCSW, you will play a crucial role in supporting the registered nurses in delivering high quality, clinical care and rehabilitation with patients in their own homes. You will be a valued member of the community nursing team and receive training and support on a multitude of clinical skills, in order to practice confidently and safely within the community. To enhance your profession, Central and North West London NHS Foundation Trust (CNWL) are investing in apprenticeship programmes and are keen to offer the opportunity for HCSW to develop into Nursing Associate Band 4 post holders.
For this position you will need to be a confident individual who is able to build good relationships with patients, their families and carers; and work in partnership with the Multi-disciplinary Team. Previous Community experience is not essential, but you will need to demonstrate that you have any understanding and awareness of autonomous working in a community setting. We are looking for people with previous experience of caring and ideally you will have a care qualification such as NVQ/QCF Level 3 or be able to demonstrate considerable experience in this field. We would expect you to be competent to work on your own, under the supervision of nurses and expect that you will have some clinical skills and knowledge.

As the position will require you to visit various locations throughout our supported area, an essential criteria would be for a valid UK driving licence as well as access to a car enabling to travel around the borough of Hillingdon.

Essential criteria
- NVQ 2/3 or equivalent
- GCSE English and Mathematics level C or above

- Foundation degree in nursing or equivalent

- Previous experience of working with a multidisciplinary team
- Experience of providing care in a clinic setting
- Experience of working with older people

- Experience of working with people with dementia or other cognitive impairments
