Trócaire Somalia’s Health and Nutrition Programme

Trócaire Somalia’s Health and Nutrition Programme is recruiting a Community Health Officer (CHO) who will be responsible for support integrated community health services across iCCM/iCCM+, primary and secondary health-care services both at facility and community level including outreach mobile clinic teams and CHWs. The CHO will support strengthening the inter-linkage between communities, health facilities, and outreach services through effective referral, follow-up, and continuity-of-care systems. The CHO will work closely with iCCM/iCCM+ supervisors, CHWs, health facility staff, mobile outreach teams, and community structures to ensure that health services are delivered in an integrated and community-centred way.

The CHO will also support school health activities through mobile outreach teams for using schools as an entry point for health promotion, disease prevention, identification of health and nutrition needs, immunisation catch-up, Vitamin A supplementation, deworming, referrals, and follow-up. The CHO will also maintain strong coordination with the Ministry of Health/District Health Office (DHO), District Health Board (DHB), Community Health Committees (CHCs), school education committees, and other community structures.

For the candidate with the required experience and passion for the role, Trócaire offers a competitive salary and benefits package. The role is a local position based in Gedo, Somalia. This position is open to SOMALI Nationals only.

Project Management

  • Supervise and provide technical and operational support to iCCM/iCCM+ supervisors and CHWs delivering integrated community health services.
  • Support implementation of school health activities through mobile outreach teams, using schools as an entry point for integrated health promotion and service linkage.
  • Support the effective implementation of iCCM/iCCM+ services and primary and secondary health-care linkages through static health facilities, community platforms and mobile outreach teams.
  • Work with iCCM/iCCM+ supervisors, facility staff and outreach teams to identify appropriate locations and service delivery points for integrated community health services.
  • Develop monthly implementation plans and activity schedules aligned with the programme workplan, community needs and MoH priorities.
  • Establish and maintain functional referral pathways linking CHWs, iCCM/iCCM+ services, mobile outreach teams, primary health facilities and secondary health facilities.
  • Coordinate the implementation of community-based, facility-based and outreach activities to ensure that services are complementary and avoid duplication.
  • Ensure that programme activities are implemented according to approved plans, technical standards, budgets, safeguarding requirements and organisational policies.
  • Identify, document and immediately report any challenges or risks that may affect achievement of programme objectives to the Community Health/iCCM Project Coordinator.
  • Conduct regular field supervision visits to assess programme implementation, provide constructive feedback and address identified technical or operational gaps.
  • Facilitate regular coordination and review meetings with iCCM supervisors, CHWs, health facility staff, mobile outreach teams and community structures.
  • Ensure that gender, inclusion, safeguarding and protection considerations are incorporated throughout programme implementation.
  • Promote systematic integration of Health, Nutrition, WASH and Protection within community, facility and outreach interventions.
  • Document achievements, challenges, lessons learned and good practices and support their integration into programme planning and implementation.

Logistics & Procurement:

  • Work with the Procurement and Warehousing team to prepare and execute a procurement plan to ensure favourable lead time for the procurement team.

Monitoring and Evaluation:

  • Work with the Trocaire management team in the field in monitoring and reporting progress and adopt agreed changes in project design to conform to varying contexts.
  • Monitor referral patterns and identify barriers affecting access to appropriate care.
  • Develop a monthly activity schedule based on the project work plan to facilitate timely implementation.
  • In collaboration with the MEAL team, set up programme impact/process monitoring systems and collect relevant secondary data relating to disease patterns, and other health indicators.
  • Contribute to the preparation of monitoring and assessment tools in collaboration with the MEAL team.
  • Prepare and submit timely weekly/monthly reports to the Community Health/iCCM Project Coordinator.

Coordination

  • Strengthen relationships between Trocaire, UN agencies, other agencies and beneficiaries (community leaders and government agencies (MOH)), working closely with all project stakeholders for the integration of services.

Qualifications:

  • University degree or higher qualification in Community Health or have a nursing/medical background with experience in public health programmes

Experience:

  • Minimum two-year professional experience in implementing and supervising community health mobilisation projects and coordinating Child Care Programs.
  • Proven track record in crafting community mobilisation, sensitisation, and awareness-raising activities.
  • Training and capacity-building skills.
  • Knowledge and experience in undertaking assessments and analysing the results.
  • Fluency in written, spoken English and Somali languages is mandatory.
  • Knowledge of different working standards and ethics, including SPHERE.
  • Demonstrable experience in managing teams, supervising and training others.
  • Ability to prioritise and commit to meeting deadlines.
  • Experience/knowledge of MoH systems, particularly at the regional and district level.
  • The candidate must be a resident of Gedo and familiar with the local and operational context in Gedo.

Skills:

  • Leadership and interpersonal skills including the ability to listen; negotiation skills at different levels; flexible and adaptable to work in unstable environments and under pressure with limited support.
  • Capacity Building skills.
  • Good computer skills (Microsoft Office).
  • Strong problem-solving skills and analytical skill.
