Main Responsibilities

As a Community Healthcare Assistant, you will:

• Deliver high-quality nursing care to patients within their own homes and residential care settings
• Carry out delegated clinical activities in line with your training, competencies and agreed care plans
• Support patients with wound care and dressings, blood glucose monitoring, continence care and other appropriate clinical interventions
• Undertake phlebotomy following appropriate training and competency assessment
• Observe patients and identify changes in their condition, reporting concerns promptly to the appropriate nurse
• Organise and prioritise your delegated visits according to patient needs
• Work independently in the community while recognising when advice or additional clinical support is required
• Maintain accurate, clear and timely patient records, including electronic patient records
• Provide compassionate support and communicate effectively with patients, carers and families
• Work collaboratively with District Nurses, Senior Nurses and other members of the multidisciplinary team
• Contribute to patient safety by recognising and reporting incidents, risks or concerns appropriately
• Participate in team meetings, clinical supervision, training, appraisals and ongoing professional development
• Support the induction and orientation of new colleagues and students where appropriate
• Assist with administrative and clerical activities required to support the effective running of the service
• Help ensure equipment and resources are maintained and used effectively
• Maintain patient dignity, privacy and confidentiality at all times

The Ideal Candidate

You’ll be a compassionate, reliable and proactive Healthcare Assistant who enjoys working directly with patients and appreciates the variety and independence that comes with community-based care.

You’ll need to be confident working on your own during delegated visits while also understanding that you’re part of a wider clinical team and knowing when something needs to be escalated.

You will have:

• NVQ Level 2 in Care or an equivalent relevant qualification
• Previous experience working within health or social care
• Strong communication and interpersonal skills
• The ability to communicate with patients and carers in a compassionate, respectful and reassuring way
• The ability to work effectively both independently and as part of a team
• Good organisation and prioritisation skills
• Confidence using IT systems and electronic patient records
• The ability to recognise changes in a patient’s needs or condition and communicate these appropriately
• A willingness to undertake training and develop your clinical competencies
• The flexibility to work across a seven-day service
• An interest in developing your career within community nursing
