Develop beyond traditional homecare and help deliver safe, nurse-led healthcare to people in their own homes.

The role

Nexus Care Services is recruiting caring and dependable Community Healthcare Assistants to support people with health and complex care needs in their own homes. Working under a Registered Nurse or Clinical Lead, you will provide personal care, monitor wellbeing and undertake delegated clinical tasks only after training and competency sign-off.

Key Responsibilities

  • Provide respectful personal care while promoting dignity, choice and independence
  • Support nutrition, hydration, mobility, continence and skin integrity
  • Complete competency-assessed clinical tasks such as medication support, blood glucose monitoring, enteral feeding, catheter care, oxygen support or observations, according to each care plan
  • Recognise changes or deterioration and escalate concerns immediately
  • Maintain accurate care records and follow infection-control, medicines and safeguarding procedures
  • Support hospital discharge, admission avoidance and complex care at home

Requirements

About you

  • Previous care, support-work or healthcare-assistant experience
  • Compassionate, reliable and confident communicating with individuals, families and professionals
  • Willing to complete clinical training and formal competency assessments
  • Valid driving licence, access to suitable transport and ability to travel between clients
  • Health and Social Care qualification is desirable but not essential
