Join the vibrant team at the South Nottinghamshire Adult Community Hub, where you will play a crucial role in connecting service-users, healthcare professionals, and other agencies to the right care at the right time. This dynamic position is perfect for a proactive individual who excels both independently and as part of a team.
Key Responsibilities
- Serve as the main point of contact for processing referrals into community services.
- Handle a high volume of calls from various agencies in a busy office environment.
- Process referrals and associated correspondence promptly and accurately.
- Follow up on queries and ensure patient records are updated efficiently.
- Utilise decision-making skills to signpost service users and professionals to appropriate service areas.
- Facilitate effective communication and collaboration across professional and organisational boundaries.
Working Hours
The Hub operates from 8am to 5pm, Monday to Friday, with shift patterns within these times. This role includes occasional weekend hours to manage a small number of referrals received outside normal working hours. Weekend hours are shared within the team and occur no more than once per month.

Requirements
- Proficiency in Microsoft Word and Excel.
- High levels of accuracy and attention to detail.
- Ability to work well both independently and as part of a team.
- Passion for providing quality service and a track record of adding value to the workplace.
- Confident IT skills
