An exciting opportunity has arisen for a Community Learning Disability Nurse to work within Nottingham City.
About the Role
The Intellectual Disabilities Service, Specialist Services Directorate provides specialist NHS services to people for whom mainstream services cannot meet their needs. We are seeking a Nurse to join our well-established Community Nursing Team.
Requirements
- Must be an RNLD with a Diploma or Degree level in Learning Disability Nursing
- Be registered with the NMC
- Highly motivated team player
- Demonstrate a high level of professional competence
- Ability to work autonomously and manage your own caseload
- Excellent interpersonal and communication skills
- Close work with patients, carers, external agencies, and other health and social care professionals
- Contribute to care plans alongside the risk management plans
- Ability to analyse and assist in making decisions regarding client risk and support in complex cases
- Manage a busy caseload and carry out relevant physical and mental health assessments
- Participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers
- Provide holistic advice and health education to service users, their carers, and community support agencies
- Demonstrate awareness of current issues and legislation affecting people with Intellectual Disabilities
- Access to a vehicle and essential car user status
