Band 6 Community Mental Health Nurse
We are looking for a skilled and motivated Registered Mental Health Nurse to join our Community Mental Health Wellness Team at NELFT.
This role offers the opportunity to work autonomously within a supportive multidisciplinary team, delivering high-quality, recovery-focused care to adults with complex mental health needs. You will act as a named key worker, playing a central role in assessment, care planning, and coordination of care to improve outcomes and support independence.
Key Responsibilities
- Act as a named key worker for a defined caseload, coordinating care and supporting recovery-focused outcomes
- Undertake comprehensive mental health assessments and develop, implement, and review care plans
- Deliver evidence-based interventions for individuals with complex and co-existing needs
- Safely administer, monitor, and review medication in line with NMC and Trust standards
- Assess and manage clinical risk, escalating concerns appropriately and contributing to risk management processes
- Work collaboratively with GPs, primary care, and partner agencies to support safe discharge and continuity of care
- Maintain accurate, timely clinical records to support care delivery and audit requirements
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High-Cost Area Supplement
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,870 and a maximum payment of £6,137) pro rata for part time staff.

Although we are a registered sponsor organisation, we are unable to offer sponsorship for all job roles. Please check your eligibility under the UKVI points-based system. When calculating the basic salary for sponsorship, HCAS (High-Cost Area Supplement) is not included to ensure fairness and consistency within our Trust.

Who We Are Looking For
- Registered Mental Health Nurse with current Nursing and Midwifery Council registration
- Experience working with individuals with complex mental health needs in a community setting
- Strong clinical decision-making, risk assessment, and care coordination skills
- Commitment to recovery-focused, person-centred care
- Ability to work both independently and as part of a team
