BAND 6 Mental Health Community Nurse

NELFT are seeking an enthusiastic and highly motivated Registered Mental health Nurse to complement our skill mix within the Mental Health & Wellness Team in Barking & Dagenham. The post holder will be working as a keyworker/care co-ordinator responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.
The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively support and liaise with other health and social care professionals and agencies.
Participating in the development and implementation of high standards of clinical care ensuring the service provided is needs led, compliant with national guidance and is viewed positively by service users.
The postholder will support the provision of holistic health care, as delegated within the agreed level of competency, to patients and families/carers in their own homes, including supported accommodation, and in nurse-led clinics.
MAIN DUTIES OF THE ROLE
The role encompasses treatment, advice and teaching within the post holder's professional competence, and applicants need to demonstrate a commitment to providing high-quality, evidence-based care.
The ideal candidate must be a registered nurse (Part 1) and will work as a member of the integrated community health care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals, and statutory and voluntary agencies, he/she will deliver individualised care to patients under the supervision of the District Nurse.
Essential Criteria For This Post Include
- RMN Registration with relevant professional body
- Working with adults who have complex mental health needs
- To have experience in working/liaising with multi-disciplinary teams, internal and external agencies in the provision of care
- To have experience of assessing, planning and initiate care
- Well-developed interpersonal skills, especially the ability to engage with service users and significant others in busy/high pressure environments
- Ability to work closely with the families, friends and significant others of service users in assessing need and planning intervention
- Understanding of Trust service structure and appropriate referral criteria and processes
- Excellent verbal and written communication skills.
- Car driver essential - Full UK driving licence and access to a car.

Key Responsibilities
- To be responsible for organising and prioritising own and others workload in the day to day allocation of work.
- Provide a range of intervention and assessment of patients, including those with multiple pathologies and complex mental health needs, taking into consideration the patient’s physical/cognitive/perceptual skills to identify the patient’s abilities and areas of difficulty.
- To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
- To be responsible for providing accurate records of information required by the Trust for audit purposes.
- To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
- To supervise pre-registration students.
- To employ professional curiosity in your day-to-day role and act upon any findings appropriately.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.
Probationary Period
This post will be subject to a probationary period.
High Cost Area Supplement Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME)
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
