Help shape a new approach to community mental health care across Buckinghamshire!
We're establishing a new Intensive Case Management (ICM) and Rehabilitation Pathway, bringing together specialist professionals to deliver flexible, proactive support for adults with some of the most complex mental health needs.
We're looking for a Registered Mental Health Nurse who is committed to building therapeutic relationships, working collaboratively across services and delivering recovery-focused care in the community. As part of this newly established pathway, you'll play an important role in developing a service designed around continuity, assertive outreach and person-centred support.
This is a community-based role where you'll meet people where they are, whether that's at home, in community settings or Trust premises. Rather than relying on traditional clinic appointments, you'll work proactively to engage people who may struggle to access or remain connected with services, helping to reduce relapse, prevent crisis and improve long-term outcomes.
As a Community Mental Health Nurse, you'll manage a defined caseload of adults with complex mental health needs, providing specialist nursing assessments, evidence-based interventions and personalised care planning.
Working autonomously and as part of the multidisciplinary team, you'll build strong therapeutic relationships with service users and their families, contribute specialist nursing expertise to clinical decision-making, and support people to achieve their recovery goals.
