Job Opportunity: Community Mental Health Nurse

We are looking for an enthusiastic Community Mental Health Nurse to join our Older Adult Team within Caithness.

Role Description

Your role will be assisting in managing the clinical care of patients with Mental ill health as delegated by Senior Practitioners (CPN) with adults over the age of 65 or of any age if a memory concern. It will involve the assessment; care planning, implementation and evaluation of the care plan, in partnership with the client, family and carers.

The role will involve:

  • Medication monitoring
  • Liaising with psychiatrists, Primary Care and with other Social Care services
  • Referring on to other services as appropriate
  • Being an active member of an MDT
  • Supervising students and Support Workers on occasion

Requirements

Applicants must be registered with the NMC and work within the code of conduct at all times.

  • Position: Permanent full-time
  • Hours: 36 hours working as per service requirements
