About the Role
We are looking for an enthusiastic Community Mental Health Nurse to join our Older Adult Team within Caithness.
Responsibilities
Your role will be assisting in managing the clinical care of patients with mental ill health as delegated by Senior Practitioners (CPN) with adults over the age of 65 or of any age if a memory concern. It will involve the assessment; care planning, implementation and evaluation of the care plan, in partnership with the client, family and carers.
- Medication monitoring
- Liaising with psychiatrists, Primary Care and other Social Care services
- Referring on to other services as appropriate
- Active member of an MDT
- Supervising students and Support Workers on occasion
Requirements
- Registered with the NMC
- Work within the code of conduct at all times
- Driving licence is essential (pool car may be available)
