Job Description: Mental Health Practitioner

This role is offered on a permanent basis.

If the successful candidate is Band 5, the post will be offered as full time. If the successful candidate is Band 6 then the post will be offered at 0.8 WTE (4 days per week).

An excellent opportunity has arisen for a Mental Health Practitioner in the Cheltenham, Tewksbury and North Cotswolds Assertive Outreach team.

We are looking for an enthusiastic and motivated individual to join our multi-disciplinary team. Applicants should have an interest in working with individuals who have severe and enduring mental health problems and are difficult to engage. The specific purpose is to deliver care in a collaborative manner that reflects the principles of AOT working, particularly aspects relating to recovery and social inclusion.

The Cheltenham, Tewksbury and North Cotswolds Assertive Outreach Team provides comprehensive mental health and psychosocial support to people with severe and enduring mental illnesses such as psychosis and mood disorders. The successful candidate will care co-ordinate a caseload, whilst working with the MDT and external agencies in order to promote patients recovery and quality of life. This post is open for all registered clinicians.

This is an exciting opportunity for a community mental health Practitioner who has some experience of working with adults of Working Age and caring for individuals with enduring mental health problems. The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.
