£39,959 – £48,117 per annum (pro rata)

The role
As a Community Mental Health Practitioner, you'll support adults with complex mental health needs through specialist assessment, collaborative care planning and therapeutic interventions, helping people achieve greater independence and recovery within their local community.

You'll:
- Undertake comprehensive mental health and risk assessments
- Develop and review personalised recovery and care plans
- Deliver evidence-based clinical interventions and specialist advice
- Work closely with service users, carers, GPs and partner organisations
- Support safeguarding, risk management and safe discharge planning
- Provide supervision, mentorship and support to students and junior colleagues

The Chichester Neighbourhood Mental Health Team brings together a range of experienced professionals to provide integrated, recovery-focused support for people living with complex mental health difficulties.

What we're looking for
We're seeking an experienced and compassionate practitioner who has:
- A Registered Mental Health qualification or equivalent professional registration
- Post-registration experience working with people experiencing mental health difficulties
- Experience managing complex caseloads and conducting risk assessments
- Strong partnership working and multidisciplinary team skills
- Excellent communication, clinical decision-making and leadership abilities
