Band 5 Registered Community Staff Nurse – Placed Based Team 9 Stratford upon Avon

An exciting opportunity to join a wonderful, dynamic and patient focused team providing high quality services to the local community in Stratford Upon Avon and surrounding rural villages.
Whether it be supporting a team member with new skills or attending to a patient and advising their family/carers, your role as a Community Nurse provides you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.
Could you be a Community Nurse looking for a new challenge or newly qualified Nurse? As a Registered Nurse you will contribute to leading a community nursing team within a multi professional environment.
The role requires travel around the local community. You will need to have a full driver's licence and access to a car. The service covers 8am - 6pm working 7 days per week. The shift patterns are 8am – 4pm and 10pm – 6pm full-time including weekend working.

The community staff nurse is a member of the Integrated Health Team, who undertakes professional duties in the client’s homes and clinics to ensure provision of high standard nursing care and health promotion. To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
