Community Nurse Associate Role

We have a 37.5 hour Band 4 Community Nurse Associate post in the Shore Locality District Nursing teams in Poole. You will deliver planned care to patients in the community setting.

Responsibilities

• Will support Individual development and training needs for the role and responsibilities.
• To work effectively within a large team of mixed skills when caring for complex patients in the community setting.
• Effective communication is key to this role and being aware of multi-disciplinary working.

Requirements

• Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

Key Duties

• To provide a high quality person-centred approach to care delivery within your sphere of practice always considering patients' privacy, dignity, and safety.
• To deliver planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council (NMC) and in accordance with Trust policies and protocols.
• To demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed.
• Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence-based/client-centred principles to undertake a range of delegated tasks independently.
• Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalate to senior registered practitioner as appropriate.
• To promote health and prevent ill health in service users, contributing to integrated care.
• To take responsibility for planned/defined tasks as required.
• To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Registered Nursing Associate role.
• Undertakes delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.
