Help us build a responsive, nurse-led community healthcare and complex care service across Sutton Coldfield, Lichfield, Tamworth and surrounding areas.
The role
Nexus Care Services is looking for a compassionate and clinically confident Community Nurse to support people with health and complex care needs in their own homes. You will combine direct nursing care with assessment, care planning, clinical review and oversight of delegated healthcare tasks.
Key Responsibilities
- Complete clinical assessments, care plans, risk assessments and reviews
- Deliver safe nursing interventions within your competence
- Recognise deterioration and escalate concerns promptly
- Delegate appropriate clinical tasks and assess care-staff competency
- Support hospital discharge, admission avoidance and continuity of care
- Maintain accurate records and work closely with families and multidisciplinary professionals

Requirements
About you
- Registered Nurse with current UK NMC registration
- Relevant post-registration experience in community, complex, acute or primary care
- Strong assessment, care-planning, communication and decision-making skills
- Able to work autonomously while recognising when to seek support
- Able to travel between clients across the service area
