An exciting opportunity for a Nurse Practitioner

An exciting opportunity has arisen for a highly motivated and innovative Nurse Practitioner to join the Norfolk Forensic Community Learning Disability (NFC-LD) team based at Little Plumstead Hospital.

The successful candidate will work as an influential and integral member of a diverse multi-disciplinary Learning Disability and Forensic team, working across a range of community settings in Norfolk with adults with a Learning Disability supporting them to remain living safely in community settings and reduce the need for admission to inpatient services.

A key feature of the team is its close working relationship with the on-site medium secure inpatient Learning Disability service, the Broadland Clinic supporting the transition of service users with needs relating to their risk profile and forensic histories back into the community in a timelier manner. The post holder will have the unique opportunity to work across both inpatient and community services.

The successful applicant will be an essential member of the multi-disciplinary team, working in collaboration with Medical, Psychology, Occupational Therapy, Speech, and Language Therapy in assessing the service user’s needs, devising individualized care plans and interventions, and evaluating outcomes which promote the service user’s needs and wishes throughout the process.
