Community Nurse Practitioner - 27.5 Hours Permanent

We are seeking to recruit a Community Nurse Practitioner to provide and support the provision of high-quality patient care. This is an exciting opportunity for an experienced, dynamic, enthusiastic, and motivated practitioner to develop new ways of working. They will be integral in delivering community services in different ways across the localities.

About the Role

As a Community Nurse Practitioner, you will be responsible for the assessment of the health and clinical needs of the patient, to undertake consultations with patients as an autonomous practitioner using advanced clinical practice skills and own clinical judgement to diagnose, treat, prescribe where required, refer to other services, and discharge patients.

A key part of the role will include making rapid clinical assessments of patients with undifferentiated conditions, making evidence-based treatment plans whilst aiming to ensure patients are cared for in the most appropriate place. As part of the Community Urgent Care Response Team, the practitioner will be expected to respond to patients who are experiencing crisis within a 2 hour national target. You will be required to ensure delivery of high quality patient centered care measured by clinical effectiveness, patient safety, and patient experience as well as meeting the key performance measures as required by the Trust.

Responsibilities

Integral in delivering community services
Assess health and clinical needs of patients
Undertake consultations as an autonomous practitioner
Diagnose, treat, prescribe, refer, and discharge patients
Make rapid clinical assessments of patients with undifferentiated conditions
Develop evidence-based treatment plans
Ensure patients are cared for in the most appropriate place

Requirements

Experience in service development and clinical leadership
Managed and supported by Advanced Clinical Practitioners, Hub Lead, and Medical Lead
Support to junior staff within the Dudley Clinical Hub
