We are looking for a Community Outreach & Ambulance Support Worker to join our team and provide safe, compassionate and practical support to people across our communities. This is a varied and rewarding role combining non-emergency ambulance and patient transport with community outreach, welfare checks, health promotion, harm reduction and engagement work.

Location: Based at 7a Dean & Chapter Industrial Estate, Ferryhill, County Durham, DL17 8LH, UK. The post holder will work flexibly across the community, and respond to changes in service demand, rota requirements and urgent operational priorities.

Hours: The role will operate on a flexible shift basis across a 24-hour period, including days, evenings, nights, weekends and Bank Holidays. Night work will form part of the role where required by service need, including community outreach, welfare checks and transport activity. Shift lengths may vary depending on operational requirements and may typically range from 8 to 12 hours.

Important application info: This role requires a calm, resilient, respectful and non-judgemental approach, with the ability to manage risk, communicate effectively, follow procedures, maintain accurate records and escalate concerns appropriately. Successful applicants are required to provide a DBS Check. Disclosure expense will be met by Supportive. All posts are subject to probationary period. Please be aware that we do not have a sponsor license and are therefore only able to appoint candidates who are eligible to work in the UK. You will be required to hold a full UK manual driving licence held for a minimum of three years.

What you'll be doing: As a Community Outreach & Ambulance Support Worker, you will:

  • Safely transport people to healthcare appointments, recovery hubs, rehabilitation services, treatment facilities and other agreed destinations.
  • Provide reassurance, dignity and compassionate support to people during their journeys.
  • Carry out welfare visits, wellbeing checks and engagement visits in community, accommodation and outreach settings.
  • Support people who may be rough sleeping, using substances, experiencing a crisis or disengaged from services.
  • Help people access healthcare, mental health support, housing services, recovery programmes and other community support.
  • Build trusting, professional and positive relationships with the people we support.
  • Provide signposting, supported referrals and warm handovers to appropriate services.
  • Work alongside healthcare professionals, outreach teams, emergency services, housing providers and other partner organisations.
  • Support harm reduction and health interventions following appropriate training and competency assessment.
  • Complete accurate records, journey logs, welfare notes and incident reports.
  • Recognise and appropriately escalate safeguarding, health and safety concerns.
  • Carry out dynamic risk assessments and follow relevant policies, procedures and safety measures.
  • Keep vehicles clean, safe, stocked and ready for use.
  • Represent Supportive professionally and positively within the communities we work in.
