About the Role

Whether it be supporting a team member with new skills or attending to a patient and advising their family, your role as a Community Nurse provides you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.
Could you be our Community Nurse looking for a new challenge? As a Registered Nurse, you will contribute to leading a community nursing team within a multi-professional environment. The role requires travel around the local community.
Responsibilities
- Provide high-quality care to patients in their homes.
- Develop leadership skills by supporting your team.
- Deliver diverse skills including wound care, catheter care, palliative care, IV drug administration, medication/injection administration, diabetes management, and more.
- Work within a multi-disciplinary team of District Nurses, Specialist Nurses, Community Matrons, Therapists, and Health Care Assistants.
- Collaborate with Local Authorities, Integrated Care Networks, GPs, and Acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.

Requirements
- Full driver's license and access to a car.
- Availability for 8am - 8pm working 7 days per week.
- Shift patterns are 8am – 4pm and 12pm – 8pm, including weekend working.
- Willingness to work weekends, late shifts, and bank holidays pro rata.
