About the Role
We are currently looking for a flexible and innovative Community Practitioner to join our experienced community neuro rehab multi-disciplinary team.
The post holder will be required to provide high quality Community Practitioner assessment and intervention skills to patients who have complex clinical needs as a result of Traumatic Brain Injury (TBI), to enable them to maximise their rehabilitation potential.
Key Responsibilities
- Work in a range of settings including the patients own home and the local community, as well as providing support to clients who reside in residential and nursing care settings in Northumberland.
- Service users may present with a combination of physical, cognitive, sensory, psychological, behavioural and social needs therefore a commitment to multi-disciplinary working is essential.
- Develop co-produced rehab plans with patients with TBI and refer on to internal and external specialists in accordance with the patients assessed need.
- Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, third sector partner agencies and the public.
- Regularly review rehab plans with patients and families and amend plans according to changing need.
- Consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
- Deliver evidenced based clinical interventions.
- Work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
- Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
- Provide clinical supervision to junior staff as delegated by Long Term Conditions Lead.
- Provide compassionate care that is based on empathy, kindness, respect and dignity.
- Incorporate a client centred philosophy into practice and client centred and collaborative care.

Requirements
The successful candidate will work across the entire county of Northumberland and must be able to meet the mobility requirements of the post.
